Community Pharmacy Patient Questionnaire

At Westminster Park Pharmacy Ltd we always try to provide you with
the best possible service, but we can never be perfect.
The reason we are asking you these questions is because we need you to
tell us what we are doing right and wrong so that we can improve all we do.
This short questionnaire should take just a couple of minutes to complete
andthe results are anonymous.
Some of these questions have been asked by the government as a summary
will be passed on to the PCT for evaluation.
